Health-Related Quality of Life in Liver Cirrhosis Patients and Its Correlation with Child Pugh Score

Liver cirrhosis causes a number of symptoms and complications that can have profound negative impacts on health related quality of life (HRQOL). Assessing HRQOL has become common clinical practice, as a consequence of increased survival of patients with cirrhosis. Information from other regions may not be generalized to other developing countries due to sociocultural differences. The general aim of this study was to evaluato HRQOL in liver cirrhosis patients and also correlation between HRQOL and Child Pugh score. Methods A cross sectional study was done on fifty consecutive liver cirrhosis patients admitted to Adam Malik General Hospital, Medan, Indonesia in June – August 2016. Inclusion criteria were inpatients and outpatients with liver cirrhosis, age >18 years. Patients with other chronic active medical diseases, psychiatric conditions, malignancy, liver transplantation and those unable to communicate or who declined to participate were excluded. The diagnosis of liver cirrhosis was based on clinical, biochemical, and ultrasound findings. HRQOL was assessed by using Chronic Liver Disease Questionnaire (CLDQ). The Child-Pugh score was calculated. All data were analyzed with statistical software. Result and Discussion There were 50 subjects, consisted of 27 males (54%) and 23 females (46%). Mean age was 50.62 + 12.77 years old. The most common of viral marker result was was HBsAg positive (46%). Among 50 patients, 8 (16%) were Child-Pugh class A, 9 (18%) were class B, and 33 (66%) were class C. Liver cirrhosis patients with ascites had worse quality of life in comparison to patients without ascites (p=0.001). Patients with anti HCV positif had poorer quality of life than patients with HBsAg positive (p=0,042). There was significant differences in CLDQ score among Child Pugh class A, B, and C (p<0.001), HRQOL was significantly worse in Child Pugh class B and C in comparison with Child Pugh class A, but there was no significant difference in CLDQ score between Child Pugh class B and C.
