    Uji Pengembangan, Efek Iritasi secara Histologi dan Sifat Bertahan dalam Lambung dari Matriks Kalsium Alginat-Kitosan yang Mengandung Indometasin

    Abstract
    Telah dilakukan uji pengembangan, efek iritasi secara histologi dan sifat bertahan dalam lambung dari matriks kalsium alginat-kitosan yang mengandung indometasin. Matriks dibuat dengan mencampurkan indometasin dengan natrium alginat dan kitosan sama banyak. Lalu ditambahkan mucilago amili 5% (b/v) sampai terbentuk masa yang kompak. Masa ini lalu dibulatkan menjadi butir-butir matriks dan direndam dalam larutan CaCl2 0,15 M selama 21 menit. Lalu dikeringkan selama 3 hari pada suhu kamar. Uji pengembangan secara in vitro dilakukan dengan melihat perubahan berat dan diameter matriks setelah direndam dalam medium pH 1,2, pH 6,8 dan pH berganti. Uji pengembangan secara in vivo dilakukan dengan melihat perubahan berat dan diameter matriks dalam lambung kelinci yang dibedah setelah 1, 2 dan 3 hari pemberian obat. Uji efek iritasi dilakukan terhadap 12 ekor kelinci. Kelinci-kelinci tersebut dibagi menjadi 4 kelompok dan tiap kelompok terdiri dari 3 ekor. Kelompok pertama adalah kelompok yang diberi indometasin dalam kapsul gelatin. Kelompok kedua, ketiga, dan keempat adalah kelompok yang diberi indometasin dalam matriks kalsium alginat-kitosan. Setelah 1, 2 dan 3 hari pemberian obat, kelinci-kelinci tersebut dibedah dan diamati secara makroskopik dan mikroskopik (histologi). Uji sifat bertahan dalam lambung dilakukan dengan melihat keberadaan matriks dalam lambung kelinci yang dibedah setelah 1, 2 dan 3 hari pemberian obat. Hasil yang diperoleh pada uji pengembangan secara in vitro menunjukkan bahwa matriks kalsium alginat-kitosan yang mengandung indometasin mengembang dalam semua medium tetapi lebih cepat dalam medium pH 6,8. Uji efek iritasi secara histologi menunjukkan bahwa 8 dari 9 ekor kelinci yang diberi matriks kalsium alginat-kitosan yang mengandung indometasin mengalami iritasi yang ditandai dengan penipisan epitel dan pelebaran pembuluh darah. Dari hasil uji pengembangan secara in vivo menunjukkan bahwa matriks kalsium alginatkitosan memiliki sifat bertahan dalam lambung.
     
    Swelling, irritation effect histologically, and gastroretentive property of calcium alginate-chitosan matrix containing indomethacine have been conducted. The calcium alginate-chitosan matrix containing indomethacine was made by mixing indomethacine with sodium alginate and chitosan equally and then it was made to become a compact mass with the addition of 5% (b/v) mucillage of starch (amili mucilago). After that the compact mass was formed to become spherical matrices and were immersed into the 0,15 M of CaCl2 solution for 21 minutes. Then the matrices were taken and dried at room temperature for 3 days. The in vitro swelling was done by observing the changes of weight and diameter of the matrix after been soaked in pH 1,2; pH 6,8 and changing pH of medium solutions. The in vivo swelling was done by observing the changes of weight and diameter of the matrix in rabbits’ stomach after 1, 2 and 3 days of drug giving. The study of irritation effect histologically was done to 12 rabbits. These rabbits were divided into 4 groups that consisted of 3 rabbits for each group. The 1st group was a group given indomethacine in gelatin capsule. The 2nd, 3rd and 4th group were groups given indomethacine in calcium alginate-chitosan matrix. After 1, 2 and 3 days of drug giving, these rabbits were killed and observed macroscopically and microscopically (histologically). The gastroretentive property was done by examining the position of matrix in rabbits’ stomach that were killed after 1, 2 and 3 days of drug giving. The result of the in vitro swelling showed that calcium alginate-chitosan matrix containing indomethacine swelled in all of medium solutions, but it swelled faster in pH 6,8 of medium solution. The irritation effect histologically showed that 8 among 9 rabbits which were given indomethacine in calcium alginate-chitosan matrix were irritated which were shown by the thinning of epithelium and the vein dilatation. As a result of in vivo swelling, it was concluded that calcium alginate-chitosan matrix had a gastroretentive property.
